View Extension Weekly COVID Testing FAQ

NC State Extension County Employees and CALS Off-campus Research Faculty and Staff,

If you have not already, please read the university memo from Friday, August 6, which outlines the most current NC State University policy on COVID-19 testing and vaccinations.

Beginning Monday, August 16, all faculty, staff and students who will be on campus, or at any NC State-owned or operated location across the state for any reason must provide proof of vaccination or they will be required to participate in a weekly surveillance testing program.

This policy now applies to all employees who receive a paycheck from NC State University, regardless of funding source, at off-campus sites including NC State Research Stations, Field Labs and all 101 Extension offices, even if you have previously been exempt from the policy. This expanded requirement will help keep our community healthy and should provide an enhanced sense of comfort to all students, faculty and staff as we begin a new semester.

Additionally, face coverings are now required in all indoor on- and off-campus spaces for faculty, staff, students and visitors, regardless of vaccination status. (Refer to the full memo for exceptions). This also applies to off-campus NC State-owned sites. In Extension offices, with the exception of Carteret County, we will defer to local government mask policies.

How to Upload Test Results to HealthyPack Portal (Off-Campus Testing Sites Only)

To upload your results from any off-campus testing site, scan or take a photo of your documented results and save it to your device.

You do not need to upload your results to the HealthyPack Portal if you have been tested at any of the following on-campus testing sites:

  • Witherspoon Student Center
  • Clark Dining Hall (closed as of July 9)
  • Magnolia Cottage at the Avent Ferry Complex
  • Centennial Textiles C-Store
  • ES King Village Community Center
  • Dan Allen Deck

Documentation should include:

  • Full Name or Initials
  • Date of Birth
  • Name of Laboratory or Testing Site
  • Date of Test
  • Type of Test – must be a PCR Test (no rapid tests)
  • Test results

Steps to upload test results:

  1. In the HealthyPack Portal, select COVID-19 from the Menu Options.
  2. Under “Enter External COVID-19 Lab Result” enter the date of your COVID-19 Test. Enter the result of the test. Select Submit.
  3.  A row will appear. In the last column you will see the SELECT FILE button. Select this and attach your COVID-19 results documentation from your files. Select Save.
  4. Once the file has been selected, the file name will appear. Select the green Upload button to submit the file.

Your documentation will be reviewed by a Student Health staff member. If your documentation can not be verified, you will receive a secure message from student health with further instructions.
